[state-history] .keel-state history preservation. Clone-before-write pattern: gate.sh (and Weaver judgment writes) append current .keel-state with ISO timestamp to docs/internal/weaver/keel-history.log before overwriting. Append-only log of all previous states. Enables dead reckoning from state history if context dies.
