[bugbot-findings-log] Bugbot findings TSV log created — append-only record of automated reviewer findings. Captain ordered durable tracking of Bugbot findings across PR review rounds after observing SD-190 signature defects (Right Answer, Wrong Work) surviving multiple review passes. File: docs/internal/weaver/bugbot-findings.tsv (444, TSV, 8 columns: date, pr, round, ref, class, finding, fix_commit, status). Pattern classes cross-ref slopodar entries for taxonomy linkage. Backfilled with 8 entries from PRs #386-#387 (rounds 1-2). Filepath referenced in weaver.md, architect.md, janitor.md agent files and AGENTS.md defect status section. New slopodar entry: “Right Answer, Wrong Work” — first code-domain pattern, capturing LLM tendency to assert correct outcomes via wrong causal paths. Captain’s quote on record: “this is subtle, slow but inevitable death. Beware the Phantom Greenlights.” Holding deck item added: coincidental-pass-gate-blindness (high priority) — verification fabric hole where no gate checks causal correctness of assertions.
