SD-316
[backref-density] Backref density policy: 9 mechanisms adopted to increase the ref web density between LLM↔human I/O. Operator’s observation: “without refs, data is effectively lost forever.” Adopted: (2) SD ref column in darkcat-findings.tsv, (4) curated interaction snapshots in docs/internal/signal-examples.md, (5) commit message DC-Ref trailers, (6) events spine — migrated from TSV to YAML for readability, (7) make journal daily collation target, (8) agent echo must cite SD/SO refs, (9) verbatim I/O recording for all sessions, (10) flag-and-capture via “log that” trigger. Rejected: (1) auto-append SD for DC catches — SDs reserved for explicit decisions, not data capture. Deferred: (3) session transcript dump. Lexicon additions: “log that” (capture this exchange to file), “scrub that” (remove from file, very rare), “mint” (create an SD or ref — evokes value). Lexicon note: “mark that” rejected as trigger for item 10 — collides with “mark” as general confirmation. “log that” adopted (10x better).
