[qa-sequencing-data-products] QA sequencing produces three data products: (1) Cross-Model Triangulation — 3 independent reviews of same code snapshot synthesized before fixes, L11 validation data; (2) Fix Quality — batch fix of confirmed issues, clean pre/post diff, measures false positive rate; (3) Human Delta — Operator walkthrough on fixed code, captures what all 3 models missed, L12 irreducibility thesis. Sequence: WAIT cross-model reviews → SYNTHESIZE triangulation → BATCH FIX → RE-GATE → OPERATOR WALKTHROUGH → COMPARE. Polecat defect rate: ~19% (3/16 tasks with gate-invisible defects).
